DATA PRIVACY NOTICE

                Talisay District Hospital (TDH) values and respects the privacy of its patients and clients (referred to herein as the Data Subject).  Thus, in accordance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012, TDH creates this Data Privacy Notice to inform its patients and clients with regard to the processing of personal information it collects, stores and processes by reason of or in relation to, or when necessary for the purpose to which TDH is being established.  As used herein, the term “personal information” refers to personal, sensitive, or privileged information acquired, shared, retained, or processed by Talisay District Hospital from its clients and patients.  The term may also be used interchangeably with “personal data”. 

COLLECTION AND USE OF DATA:

                 TDH will only collect and use personal information that its patients and clients chose to divulge or share including those shared or posted in Social Media or TDH Website, or thru other electronic means, as well as those CCTV Footages, and only as necessary but not limited to the following purposes:

  1. Patient Care Management
  2. Issuance of Medical Certificates, Abstracts, Live Birth, and other Certifications relating to or concerning Patient’s Medical Records
  3. Processing of Employment Applications, Retirement, Clearances, and other employment matters
  4. Procurement Process
  5. Billing and Settlement of Bills/Charges
  6. Processing of customer complaints and suggestions/ Conducting of Customer Satisfaction Survey
  7. Research purposes
  8. Availment to financial assistance programs of the government; Philhealth claims
  9. Investigation by the appropriate government authority
  10. Certification purposes
