BAC RESOLUTION 001-2012

March 8, 2012

The Talisay District Hospital –Bids and Awards Committee conducted its first competitive bidding for 2012 last February 23, 2012 for the following projects:

Lot B. Various Intravenous Fluids

Project II:

Lot A. Medical Supplies

Lot B. Dental Supplies

Lot C. Housekeeping Supplies

Project III:

Various Construction Supplies

Whereas, only 2 companies showed interest and submitted bids namely EUROMED and

INTERMED and found to be eligible.

 

Whereas, EUROMED submitted bids for Project I: Lot A Item 10 (Metronidazole 500mg

tab), Item 13(Salbutamol tab), Item 28 (Paracetamol 250/5ml), Item 30

(Salbutamol neb), Item 44 (Metronidazole 500mg vial), Item 47 (Ranitidine

25/ml amp) and Item 48 (Tramadol 50/ml amp) and found to be the only bidder

and within the lowest calculated bid.

 

Whereas, INTERMED submitted bid for Item 50 (TIG 250 IU) and found to be within the

lowest calculated bid.

 

Whereas, no bidders submitted bids for Item 1 (Azithromycin 500mg tab), Item 2

(Captopril 25mg tab), Item 3 (Cefixime 100mg cap), Item 4 (Cefuroxime 500mg

tab), Item 5 (Ciprofloxacin 500mg tab), Item 6 (Ceterizine 10mg tab), Item 7

(Clarithromycin 500mg tab), Item 8 (Ferrous SO4 + Folic acid), Item 9

(Mefenamic acid 500mg tab), Item 11 (Nifedipine 5mg cap), Item 12 (Omeprazole

20mg cap), Item 14 (AlMg(OH) bot), Item 15 (Amoxicillin 250/5), Item 16

(Amoxicillin 100/ml bot), Item 17 (Azithromycin 200/5 bot), Item 18 (Cefalexin

100mg bot), Item 19 (Cefalexin 250/5 bot), Item 20 (Cefuroxime 250/5), Item 21

(Clarithromycin 125mg bot), Item 22 (Cotrimoxazole 200/40 bot), Item 23

(Ipatropium + Salbutamol neb), Item 24 (Mefenamic 50mg bot), Item 25

(Metronidazole 120mg bot), Item 26 (ORS), Item 27 (Paracetamol 100mg bot),

Item 29 (Paracetamol 125mg supp), Item 31 (Amikacin 250mg vial), Item 32

(Ampicillin 250mg vial), Item 33 (Ampicillin 1gm vial), Item 34 (Aminophylline

amp), Item 35 (Cefazolin 1 gm vial), Item 36 (Cefuroxime 750mg vial), Item 37

(Ceftriaxone 1 gm vial), Item 38 (Hydrocortisone 250mg vial), Item 39

(Hydrocortisone 100mg vial), Item 40 (Hyoscine N-butyl bromide amp), Item 41

(Gentamycin 40mg vial), Item 42 (Hydralazine amp), Item 43 (Lidocaine with

Epinephrine carpule), Item 45 (Oxytocin amp), Item 46 (Phytomenadione amp)

and Item 49 (Tetanus Toxoid).

 

Whereas, only EUROMED submitted bids for Lot B IV fluids for Item 1 (Sterile Water

for Injection), Item 2 (D5 0.3NaCl 500ml), Item 5 (D5LR 1L), Item 6 (Balance

Multiple Maintenance 1L), Item 7 (Balance Multiple Maintenance 500ml),

Item 8 (NSS 1L) and Item 9 (PLR 1L) and is responsive.

 

Whereas, both EUROMED and INTERMED submitted bids for Item 3(D5 0.3%NaCl 1L),

Item 4 (D5LR 500ml) and Item 10 (D5W 500ml) and found EUROMED as the

lowest bidder.

 

Whereas, no bidder submitted bids for Project II and Project III.

 

The Bids and awards Committee thus pass the following resolutions and hereby resolved and recommended:

 

1. To award EUROMED for

a. Project I: Lot A

Item 10 (Metronidazole 500mg tab)

Item 13(Salbutamol tab)

Item 28 (Paracetamol 250/5ml)

Item 30 (Salbutamol neb)

Item 44 (Metronidazole 500mg vial)

Item 47 (Ranitidine 25/ml amp) and

Item 48 (Tramadol 50/ml amp)

b. Lot B IV fluids

Item 1 (Sterile Water for Injection)

Item 2 (D5 0.3NaCl 500ml)

Item 5 (D5LR 1L)

Item 6 (Balance Multiple Maintenance 1L)

Item 7 (Balance Multiple Maintenance 500ml)

Item 8 (NSS 1L)

Item 9 (PLR 1L)

Item 3(D5 0.3%NaCl 1L)

Item 4 (D5LR 500ml) and

Item 10 (D5W 500ml)

 

2. To award INTERMED Project I Lot A. Item 50 (TIG 250 IU prefilled syringe).

3. To declare failure of bidding to other items on Project I Lot A as no bidder submitted bids.

4. To declare failure of bidding on Project II and III as no bidder showed interest.

5. That a re-bidding shall be conducted and is set on the following schedule:

ITB: March 15, 2012

Bid opening: April 3, 2012

Bid evaluation: April 10, 2012

Postqualification: April 11, 2012

 

 

Therefore, we forward to the Chief of Hospital for the approval of this matter.

3.0  SUBMISSION AND OPENING OF BIDS AND PRELIMINARY EXAMINATION

The submission of bids took place as scheduled on February 23, 2012 at 1:00 p.m.  Two (2)

prospective bidders who procured the bidding documents submitted their respective bid proposals, namely:

1. EUROMED

2. INTERMED

The first envelope containing the Eligibility Documents: Legal, Technical, and Financial Documents

were individually opened.  The presence, completeness and correctness of the required documents were

checked and verified by the BAC in the presence of the prospective bidder's representative.  Upon checking

the two (2) bidders were declared eligible.
